	AddictionHealingCoach.com Rates and Terms of Use Agreement Read This Terms of Use Agreement Before Accessing Website. Effective Date: This Terms of Use Agreement was last updated on April 5, 2009 This Terms of Use Agreement sets forth the standards of use of the AddictionHealingCoach.com Online Service for Registered Members. By using the AddictionHealingCoach.com website you (the “Member”) agree to these terms and conditions. If you do not agree to the terms and conditions of this agreement, you should immediately cease all usage of this website. We reserve the right, at any time, to modify, alter, or update the terms and conditions of this agreement without prior notice. Modifications shall become effective immediately upon being posted at AddictionHealingCoach.com website. Your continued use of the Service after amendments are posted constitutes an acknowledgement and acceptance of the Agreement and its modifications. Except as provided in this paragraph, this Agreement may not be amended. 1. Description of Service AddictionHealingCoach.com is providing addiction coaching to assist in addiction healing. Clients must provide (1) all equipment necessary for their own Internet connection, including computer, modem and telephone service (2) provide for client’s access to the Internet, and (3) pay any fees relate with such connection. 2. Payment Methods: We accept Visa and Mastercard. Credit Card information is collected and payment made prior to or at the beginning of each session. 3. Rescheduling, Cancellations and Missed Appointments: If you are unable to keep your scheduled appointment we require a 24 hour notice of cancellation. For twenty four hour notification of missed appointments, no fee will be charged. For rescheduled, cancelled or missed appointments without a twenty-four hour notice, your credit card will be billed for the full length of the session for $75. 4. Rates: AddictionHealingCoach standard fee for service is $75 for a forty-five minute session. For every 5 minutes over your standard 45 minute session a fee of $7.00 will be charged. 5. Confidentiality: Although, this is not psychotherapy we do take every precaution to insure confidentiality. However, if you choose to communicate through cell phone, email or internet, we cannot guarantee protection of you personal health information and sessions. 6. Disclaimer of Warranties. The site is provided by AddictionHealingCoach.com on an “as is” and on an “as available” basis. To the fullest extent permitted by applicable law, AddictionHealingCoach.com makes no representations or warranties of any kind, express or implied, regarding the use or the results of this web site in terms of its correctness, accuracy, reliability, or otherwise. AddictionHealingCoach.com shall have no liability for any interruptions in the use of this Website.
